Effective October 1, Council on Aging is a contracted provider of CareSource to provide care management for people enrolled in the Ohio Home Care Waiver and Specialized Recovery Services Program (SRS). Council on Aging has been providing care management services for Ohio Home Care clients since 2014 and SRS clients since 2016.
Ohio Home Care is an in-home long-term care option for eligible children and adults who have significant disabilities and/or mental health needs. It provides cost-effective home and community-based services to help people remain in a community setting, rather than a nursing home.
SRS provides specialized support for adults with a serious and persistent mental illness, a diagnosed chronic condition, or who are active on the solid organ or soft tissue transplant waiting list.
The programs are administered by the Ohio Department of Medicaid (ODM). They preserve independence by giving participants greater control and choice over their care, including where they receive care, the types of services they receive and who provides their services.
Earlier this year, CareSource was awarded one of two statewide contracts issued by ODM to provide care management services via the Ohio Home Care and SRS programs. COA agreed to serve as CareSource’s sub-contractor to provide case management services to Ohio Home Care and SRS consumers in the Cincinnati/Dayton region.
