Council on Aging’s 2016 Annual Report includes data, photos and personal stories that highlight our mission to enhance people’s lives by assisting them to remain independent at home through a range of quality services.
The report focuses on programs – both new and old – that help us achieve this mission. For example, a new program, Specialized Recovery Services, promotes independence through supportive services for adults with mental illness. Long-standing programs, such as the Elderly Services Programs, offer a life-line for thousands of area seniors who need in-home care services but do not qualify for Medicaid.
COA’s report highlights our increasing role in the region’s health care landscape, including MyCare Ohio (the state’s integrated care system for people enrolled in Medicare and Medicaid), and hospital-based programs such as Care Transitions and Fast Track Home. Through these programs, we are working to strengthen the connection between health care and long-term care.
As the Area Agency on Aging for Butler, Clermont, Clinton, Hamilton and Warren counties, Council on Aging is in our community every day, working with seniors, people with disabilities, their families and caregivers. In 2016, COA:
- Provided support to nearly 20,000 people to help them remain independent in their homes
- Conducted Care Transitions InterventionsTM with more than 7,300 Medicare patients at risk for hospital readmission
- Responded to 45,822 requests for information and referral to community resources
- Contracted with 228 organizations to provide more than one million hours of home care services
- Provided more than 1.5 million meals to seniors via congregate and home-delivered meals programs
- Provided 212,222 transportation trips for medical and senior center activities
The report includes other highlights of 2016, such as the passage of two senior services tax levies by record-breaking margins, and the success of our Care Transitions Program.
In addition to our organization annual report, we also produce annual reports for the Elderly Services Programs in Butler, Clinton, Hamilton, and Warren counties. These reports include client stories and photos along with detailed demographic and financial information, COA administers the program via contracts with county commissioners.
